Are you or your loved one a Medicaid participant? Did you know that you could qualify for services in your home to help you maintain your independence? Michigan’s MI Choice Waiver Program provides home and community-based services (HCBS) for state residents who are elderly or physically disabled and at risk of being institutionalized (being placed in a nursing home). A variety of benefits are available, and based on an individual care plan, program participants receive long-term services and supports to enable them to continue to live independently. Benefits might include home modifications, adult day care, meal delivery, personal emergency response systems, personal care assistance, and nursing services. Persons can live in their home, the home of a relative, an adult foster care home, or a home for the aged, which can be thought of as assisted living.
We here at Health Care At Home can assist you to get started on the process to become a MI Choice Waiver participant. As a MI Choice Waiver service provider for both the Region VII Area Agency on Aging and A&D Waiver we can be your provider for services after enrollment in Huron, Tuscola and Sanilac Counties. If you have a family member or friend that you would like to have as your primary caregiver we will work with you to enable them to become an employee of Health Care At Home as your designated care aide.
